Questionnaire

If you wish to arrange an initial assessment by a therapist, you may optionally download and print off a questionnaire, then complete it and return it to us by post. This will allow you to tell us in confidence a little about your life and the difficulties you are experiencing, so that we can move forward more quickly with any therapy that may follow.

Confidentiality

In order to preserve confidentiality we do not ask for details of your name and address on the form, so before you return it to us please telephone the Service Administrator to give these details and to ask for a confidential registration code. We will use the code to link your returned questionnaire to your name once it has arrived at Hampden House.

All personal details are kept in secure locked filing systems.
